Important points to read before completing the referral form below:

• From the point of completing our forms the wait is between 3-6 months for an appointment.

• After you have submitted the form we will send you a consent for with information about our services, an initial survey and some online assessments to complete to give us more information.

Using the form:

• If you are an adult completing this on a child's behalf please select "Someone else" and seeking support for a "child".

• When entering your phone number please use the +44 format and remove the 0 at the start of your number (example: 07876 543 321 becomes +447876 543 321). If this is not correct you will not receive texts.

• Communication will come from our system called 'tacklit'.

• Please check your junk folder as sometimes our response can get sent there.

• There is currently no confirmation when you submit the form. Within a week of submitting your form we will send out information to you and this is confirmation that we have received your request. If you haven't heard from us after a week please pop us an email to check we have received your referral.
